If you have decided to donate your organs and tissue this is what you need to do…
1. It’s extremely important that you make sure your family knows about your intentions well in advance of your death, so that they will honour your commitment at a difficult time.
2. Download an organ donor card below – or call 1.866.949.0003 and ask us to mail you a card.
3. Complete the Donor ID Card and place it in your wallet alongside your personal identification.
4. Give the Donor Notification Card to your loved ones.

Ontario
Ontario residents have the option of joining a provincial organ donor registry run by the Ontario Ministry of Health and Long Term Care. Join here. **Important Note: Please return your form to the address provided on the registration form – do not send it to us.
Alberta, North West Territories, Nova Scotia and Nunavut
If you live in Alberta, NWT, Nova Scotia or Nunavut you have the option of signing the back of your provincial Health Care Card.
British Columbia and Yukon
Both BC and Yukon have an organ donor registry that residents can join. For more information please call 1.866.949.0003.
Manitoba, Newfoundland and Labrador
If you live in Manitoba, Newfoundland or Labrador you can sign the back of your driver’s license.
New Brunswick
In New Brunswick you can register with Medicare.
Prince Edward Island
To become an organ donor in PEI you can get a red heart put on your drivers license, as well as, a sticker for your PEI Health Card.
Quebec
Quebec has a sticker to be placed on your Health Insurance Card.
Saskatchewan
Show your consent in Saskatchewan by placing an orange ‘organ and tissue donor’ sticker on your Saskatchewan Health Services Card.
